To obtain a hard copy of your Gunnison Valley Health medical records, please
complete the appropriate online or paper form below. If choosing the paper
form, please complete all sections of the form before delivering, faxing
or emailing it to our team. Behavioral health records can only be requested
via paper form at this time.
Please specify if you would like to pick up the medical records in person,
have them faxed, emailed or mailed to you.
Requests for medical records are processed in the order that they are received.
Medical records will be delivered within 10 to 14 days upon receipt of
the request.

Gunnison Valley Health provides patients access to specific health information
via an online medical record.
If you are registered as a patient at Gunnison Valley Hospital, you will
be able to view the following information online upon discharge:

Problem List
Active/Current Medication List
Active/Current Medication Allergy List
Allergy List
Care Plan
A limited number of same-day test and procedure results will also be available
online. Information from visits to physician offices are not available
here. Please contact the physicians involved in your care for medical records.
